Release Of Liability & Assumption Of Risk For Students

I am participating as a student in Yoga classes at Mysore Houston, DBA (herein referred to as “MH”). I am aware that participating in Yoga classes requires physical exertion that may cause physical injury, and I am aware of the risks and hazards involved in practicing Yoga. I am in good health and suffer from no physical impairment that would limit my participation in these classes. I understand that it is my responsibility to consult with a physician prior to, and regarding my, participation in these classes.

I know that attending classes at MH involves the risk of injury to me or my guest, whether I or someone else causes it. I know that specific risks vary from activity to activity and the risks range from minor to major injuries. I also know that practicing Yoga at MH involves physical adjustments to my body by teachers or teaching assistants. By executing this waiver, I consent to being touched by any teacher or teaching assistant at MH. I understand and acknowledge that I must orally inform any teacher or teaching assistant prior to class if I do not want to be physically touched during my Yoga practice.

In consideration of my participation in activities offered by MH, I understand and agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of using MH facilities or participating in MH classes. I agree to hold MH, and the landlord/owner of any premises where such classes are held, their employees and agents, harmless from the same. I agree that MH, its employees and agents, and the landlord/owner of any premises where such classes are held, shall not be held liable for any claim, demand, or cause of action of any kind on account of death, personal injury, economic loss, property damage or loss of any kind resulting from, or related to, my participation in MH classes or use of the MH facilities or property on which MH classes are housed. This waiver of claims includes my knowing and voluntary release of any and all causes of actions, including negligence and other tort claims, against Mysore Houston, DBA, its agents, and employees, whether such claims are related to my physical activity or not.

I acknowledge that I may revoke this waiver at any time by submitting my revocation in writing to MH, but that in doing so I will be prohibited from entering the MH premises or practicing Yoga at MH.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
